UCP Supported Living
(​"UCP Foundations")

If you're an adult who experiences developmental disabilities, you can use our Supported Living services to live independently in your own house or apartment.

​All of our supports are one-on-one... and completely customized. Need a quick, weekly check-in? We can do that! Need a team of round-the-clock caregivers? We can do that, too. We can also offer assistance with household management and social activities. 

It's all about giving you the supports you need so that you can live how, and where, you want!

Who Is Eligible?

  • Adults 18 years of age or older who are receiving developmental disabilities services through a county developmental disability program or local brokerage, which provide access to funding. If you are unsure if you qualify, please feel free to contact our team.​
  • We can also accept private pay and other individual payment arrangements.

social supports:

  • ​​We can assist you with community involvement, such as pursuing your interests, meeting people, and serving as a self-advocate.
  • ​Want to take an awesome vacation? Attend a family reunion? Go to college? We can assist with goal planning. ​​
  • We can also provide emotional support, talking through feelings, decisions, crises, or anything else that comes up. ​24-hour emergency response is available.

in-home supports:

  • We can assist with meal preparation, laundry, cleaning, and/or household organization. We can also assist you to learn to do these tasks for yourself.
  • We can assist with personal care. 
  • ​We can also assist you to move, buy a home, or communicate with a landlord.

medical supports:

  • We can assist with medication and monitoring on-going health concerns​.
  • We can make doctor's appointments and provide transportation.
  • We can also assist you to make sure your voice is heard by medical professionals.

household Management:

  • We can assist you to track your bills, make payments, create budgets, and visit the bank.
  • ​We can assist you to manage benefits and pay taxes.
