UCP Oregon's Brokerage Services

Circa September, 2021, UCP Oregon is running two separate brokerages: our original brokerage ("UCP Connections") and a new brokerage (UCP Mentors").

Both are called "support services brokerages."

​That means we assist adults who experience disabilities to achieve life goals, access resources, and connect to service providers.

It's All About You:

  • Our Personal Agents will get to know you, your life goals, and your supports through a "person-centered" planning process.
  • Then we'll assist as you (and the people in your life) take action.

  • Our Personal Agents will be there to support you and and celebrate you!
  • We will assist you to navigate through a complex service system.
  • We can also assist with any personal crises that might come up.​

access to great resources:

  • Do you need funding, community resources, or in-home supports? We know exactly where to look.
  • Our goal is to find exactly the right mix of people and resources for you!

Frequently Asked Questions:

WHO IS ELIGIBLE?

  • You must be 18 years of age or older.
  • You must be deemed eligible for Intellectual/Developmental Disability services through your local County and Developmental Disabilities Program office.
  • You must live in Multnomah or Clackamas county.​​​

HOW DO I ENROLL?

  • Contact your local County Developmental Disabilities Program office to request in-home supports through UCP Connections.
