Once you’re enrolled in Supported Living, you’ll have access to one-on-one consistent staffing and schedule, a UCP Coordinator who knows you well, and an entire department available to respond to your needs, goals and interests.

24/7 Backup:
- Supports built around your preferences—not a set schedule. Whether the need involves 24-hour, round-the-clock support or a few hours a week, we build a team around what you actually need and want.
- UCP supports independence without making assumptions or overstepping.

- A UCP coordinator who listens and helps direct your service plan the way you want.
- A schedule that you can count on, every week.
- 24-hour emergency backup, always.

What’s a UCP Supported Living Coordinator?
Your Coordinator is one of the things that makes Supported Living at UCP different.
- They carry a very small caseload—so you get real attention and real-time response.
- They know your preferences, your history, and your goals. When something changes, you only have to tell them once. They handle the rest: updating your plan, training your staff, and making sure everything stays on track.
Whole Life Support:
Supported Living at UCP Oregon looks different for each person. That can include just about anything, including:
- Maintaining your home: cooking, cleaning, and household routines.
- Supporting your health: medications, medical appointments, and personal care.
- Getting around town: transportation and community activities.
- Help with your finances: budgeting, bills, and errands.
- Living the life you want: support to pursue the hobbies, friendships, and activities you care about.
24/7 Backup:
- A manager carries our emergency pager at all hours, every day. You're never without backup—and neither is your team.
Eligibility:
- Live in Multnomah or Clackamas County.
- Are 18 or older.
- Are living in your own apartment or house, or with your family, or without permanent housing at this time.
- Have a documented intellectual or developmental disability.
- Are eligible for Medicaid.
